2026-27 FinalForms registration is open

Welcome to the 2026-27 Athletic Year! We need to ensure all necessary paperwork is completed for your athlete's participation.

Here are the steps you should take:

Log into Your Athlete's Account/Profile: Access your athlete's account or profile by clicking this LINK sports registration and paperwork are managed.

Complete Required Paperwork: Fill out all the required forms and paperwork for the upcoming sports season. This typically includes consent forms, medical information, and possibly other documentation depending on the sport and school requirements. Please ensure that all neccesary forms are in the "green", which means they are all properly completed and submitted.

Select the Sport: Choose the specific sport(s) that your athlete is interested in participating in for the 2026-27 season. This helps the athletic department, and coaches plan accordingly.

Submit Sports Physical: Ensure that your athlete's physical is up-to-date and submitted as required. This is crucial for eligibility and ensuring your athlete is physically fit to participate safely. Physicals can be uploaded to FinalForms, emailed to rachel.breckenridge@ndcl.org, or dropped off at the NDCL Athletic Office. The physical must be the OHSAA pre-participation physical form, it can be found on our athletics website by hovering over "Forms" at the top of the page and can be downloaded to your device. Your student-athlete must have an active physical on file in FinalForms before being able to participate in their team's first official practice of the season. Physicals will need to be signed and dated by a physician.
